  • Personal care assistance and daily living activities
  • Transportation to medical appointments and community activities
  • Social and recreational programming

Our website provides information about our services, allows for appointment scheduling, contact forms, and investor/donor opportunities.

3. Admission and Residency Terms

3.1 Admission Requirements

Admission to Harmony House Group Home is subject to:

  • Completion of application and assessment process
  • Medical evaluation and documentation of care needs
  • Verification of insurance or payment arrangements
  • Background screening as required by state regulations
  • Availability of appropriate care services and bed space
  • Execution of Residency Agreement

3.2 Residency Agreement

All residents or their legal representatives must sign a separate Residency Agreement that outlines specific terms including care services, fees, payment terms, rights and responsibilities, and termination procedures. The Residency Agreement is incorporated into these Terms by reference.

3.3 Right to Refuse or Discharge

We reserve the right to refuse admission or discharge a resident if:

  • The resident's care needs exceed our capability or licensing
  • The resident poses a threat to themselves or others
  • Payment obligations are not met
  • The resident or family violates the Residency Agreement or facility policies
  • Required by law or regulation

Discharge procedures will comply with all applicable state and federal regulations, including appropriate notice and transition planning.

4. Payment Terms and Financial Obligations

4.1 Fees and Charges

Residents or their responsible parties are obligated to pay all fees for services as outlined in the Residency Agreement. Fees may include:

  • Monthly base care fees
  • Additional service fees for specialized care
  • One-time admission or application fees
  • Personal supply and incidental charges

4.2 Payment Methods

We accept payment via check, credit/debit card, electronic transfer, and direct billing to insurance or government programs (Medicaid, Medicare). Payment is due in advance of services unless otherwise agreed in writing.

4.3 Late Payment

Late payments may result in late fees, suspension of non-essential services, or termination of residency in accordance with state regulations and the Residency Agreement.

4.4 Insurance and Benefits

We will assist in coordinating payment through insurance or government benefits programs. However, residents are ultimately responsible for payment regardless of insurance coverage. Any balance not covered by insurance or benefits is the resident's responsibility.

5. Facility Rules and Code of Conduct

All residents, visitors, and staff must comply with facility rules and policies, including:

  • Respect for other residents, staff, and visitors
  • Compliance with medication and treatment plans
  • No violence, threatening behavior, or harassment
  • No illegal drugs, weapons, or contraband
  • Observance of visiting hours and security procedures
  • Adherence to smoking and alcohol policies
  • Care and respect for facility property

Violation of facility rules may result in visitor restrictions, loss of privileges, or termination of residency as permitted by law.
